Verification
Students are randomly selected for verification by the Department of Education. The more applications a student submits the greater their chance of being selected for verification. Students selected for verification are required to provide the documents listed below to complete this process:

1011 Verification of Household and Income Worksheet - printable version

1011 Request for Consideration of Independent Status - printable version
3. Signed copy of your 2009 1040 tax returns - Internal Revenue
Service (IRS) website or call the IRS at 1-800-829-1040.
How to determine which portion of the verification worksheet you need to complete?
If a student can answer yes to at least one of the following questions, that student needs to complete the Independent portion of the worksheet (front and back). The Department of Education requires verification of the members in your household
1. Were you born before January 1, 1986?
2. Are you working on a masters or doctorate program?
3. Are you married?
4. Do you have children that you financially support?
5. Do you have dependants other than your children or spouse that
you provide more than 50% of their support?
6. Are you an orphan or ward of the court?
7. Are you a veteran of the U.S. armed forces?
If a student can not answer yes to at least one of the questions listed above, that student needs to complete the Dependant portion of the worksheet (front and back).

A copy of your 2009 Federal 1040
tax return is required. If you do not have a copy, contact the
IRS at 1-800-829-1040 and request a 1040 and W2 transcript.
